This form can be completed online and then printed out. Then bring the form to the Theme Office.

Lab Rotation Form

This form is to be taken with the student when meeting with rotation mentor for first meeting. Mentor is responsible for completing the form with the expectations he/she has for the student for the rotation. Student and mentor will sign the form, student will keep the form until end of rotation. At end of rotation, student and mentor will meet for a final meeting. Mentor will complete the form, along with a grade. Student (or mentor) will return the signed form to the Theme Office

Student Evaluation of Lab Rotation Form

This form is to be completed by the student after each rotation. The student is to honestly evaluate his/her experience in each rotation. Forms will be held confidential until after grades are in for the semester the evaluation covers.
