FAQ Tuition Remission for SOM

• Carefully read the instructions and accurately complete the Affidavit of Financial Support form.
• Ensure that your total funds meet the minimum financial obligation.
• If you leave the form blank or do not return the form, you will be required to submit a new one before your visa can be processed.
• If you have a sponsor, the sponsor's signature is required.
• Indicate a UAB assistantship only if you have already been awarded the departmental assistantship.

• Must be an original with the stamp/seal of the bank and an official bank signature.
• Bank statement must be an original. Photocopied, faxed or scanned copies will not be accepted.
• Must be in English or have a certified English translation.
• Date on statement cannot be more than 1 year old.
• Must indicate "deposit balance" in US$.
• Must include name of the bank, date statement was prepared, student's name or sponsor's name (in English).
