Ethics Commission Report for 2013The State of Alabama Ethics Commission requires that UAB employees who meet a certain salary threshold, or whose job includes management of state funds in certain capacities, complete an annual Statement of Economic Interests form. This year the salary level has been increased from $50,000 to $75,000; and anyone serving as a supervisor has been added to the list of those who are required to complete the form. As required by law, the UAB Office of Human Resources has identified you both internally and to the commission as such an employee in 2012.
In order to help you fulfill this requirement, the Ethics Commission now provides the form and instructions on their Web site, www.ethics.alabama.gov, so that you can quickly and conveniently file online. While you still can choose to print a copy of the form, complete it and return it directly to the Ethics Commission just like in previous years, we encourage you to take advantage of the new quick and cost-effective online filing option. When you do, you will receive an e-mail from the Ethics Commission (the e-mail address will contain @ethics.alabama.gov) providing you with a code that confirms your form has been filed. Keep this confirmation code on file in a safe place because you will be asked for your confirmation code when you try to view your Statement of Economic Interests information during future visits to the Ethics Commission site.
If completed online, the form must be completed and the confirmation code received by the state mandated deadline of April 30, 2013. In addition, direct mail forms also must be postmarked no later than April 30, 2013.
This year there are additional steps necessary to fully complete the Statement of Economic Interests form. The checklist below is to help ensure that you remain in compliance with state law. Please note that under Alabama law, failure to comply with the new training requirement may (1) result in a fine of up to $1,000, and (2) constitute a Class A misdemeanor.
Follow these steps to ensure that you are in compliance with state law:
Step 1: Complete ethics form. Go to http://www.ethics.alabama.gov/ to complete the Statement of Economic Interests form online. If you prefer, you can print a copy to complete and mail in.
Step 2: Print and sign the Alabama Ethics Training Certificate of Completion and send it to your dean or vice president’s office for compliance tracking. The state requires UAB and all employers to keep a printed copy of the certificate in each employee’s permanent personnel file.
