Hypertension affects approximately 76 million Americans and disproportionally impacts the African American community, significantly increasing the risk of coronary heart disease, stroke and contributing importantly to heart and kidney failure. While even a small reduction in blood pressure can result in significantly improved outcomes, only one in three persons with hypertension is able to achieve their blood pressure goal.
Help employees meet others living with hypertension, talk about what it's like to have hypertension, learn how others are dealing with their disease, and to obtain peer and professional support.
Blood pressure screening and tracking, peer group discussions and shared experience, and mentoring individuals to get and maintain their blood pressure at the goal set by their physician or recommended by the national guidelines. These activities are guided by a health care professional.
