Enroll

Enrollment in INTERMACS can be a two-step process.  First, each participating site must complete the on-line Site Enrollment process.Once a site is an INTERMACS participating member, the site can begin submitting data from consented patients through the Patient Enrollment web based data entry system.


Reminders:

  • Provide INTERMACS with annual renewal documents prior to the due date.
  • Remember to have the subject date the informed consent at the time of signature.
  • All subjects that transfer from one INTERMACS site to another must complete a new Informed Consent and a Patient Authorization for INTERMACS to Release Information form.
  • Transmit correspondence with Protected Health Information (PHI) only through a secure e-mail process.
