IT Related Contracts & Agreements

As part of the University's contract review process, UAB IT is responsible forcontract reviewing any University contract that includes an IT or IT related component prior to such contract being executed.  The information and sample addendum, below, provide details on how to ensure that your IT related contract can be routed for execution in a timely manner. Questions on the process or requirements should be directed to UAB IT

What constitutes an IT related contract or agreement? (see note below about renewals)
 
  • Contracts for software and/or software maintenance, including renewals 
  • Professional services agreements for installation of software/systems
  • Contracts where University data/information will be processed, hosted, or stored as part of the vendor's services (regardless of whether system is located at University or at the vendor's site)
  • Contracts involving acceptance/processing of credit card transactions
  • Contracts involving design, creation, maintenance, support, and/or hosting of any website/webpage
  • Contracts involving personally identifiable information (PII) or personal health information (PHI) - does not include Health System Agreements which are managed by HSIS
  • Cloud-based services of any kind
  • Other similar contracts/agreements

 

What can I provide my vendor in advance so they are aware of the contract requirements?
 
  • For most agreements the following two addendums will be added during the routing process.  You can provide those addendums to your vendor in advance for their review and acceptance/signature prior to routing at UAB and include them in the routing packet.  
    • The general UAB Addendum (found on the Financial Affairs contract website) which covers information related to State, University, and Legal issues/clarifications.  Any changes to the UAB addendum must be approved by University Contracts.
    • The UAB Information Technology Addendum which covers additional confidentiality, information security, liability, and warranty language. Portions of the IT Addendum may be applicable to only certain agreements and that applicability is detailed in the various sections of the addendum.  Any changes to the IT addendum must be reviewed and approved by UAB IT. 
  • For agreements that include HIPAA (health related information) a Business Associate Agreement (BAA) may also be required. For information related to the BAA see the UAB HIPAA website

 

What do I need to ensure is included in my routing packet to UAB Contracts?

In addition to the requirements shown on the Financial Affairs contract review process website the following items are needed to ensure a timely review:
  • For RENEWAL agreements: please ensure that a copy of the original, underlying, governing agreement that was previously executed between UAB and the vendor is included with the routing packet, including any subsequent addendums.  If that original agreement included the IT Addendum and UAB Addendum, no additional language may be necessary as long as the renewal document references the original agreement and not a new document. If the original agreement did not include the IT Addendum, please have it reviewed/executed by your vendor and attach to the renewal agreement when routing.
  • For NEW agreements: please ensure that a copy of the full, underlying, governing agreement (license, EULA, professional services, etc) and any documents referenced in such agreement such as separate maintenance, support, privacy, statement of work, etc. (on a website or otherwise) are included with the routing packet.  Submission of an order form, quote, schedule, etc. without the full underlying terms/conditions document will cause delays in the review process as those documents will be requested before the review can be completed. In addtion, please have your vendor review/execute the UAB Addendum and UAB IT Addendums and attach them to the new agreement when routing.

Oracle Software

UAB's Oracle agreement includes both site-licensed product available to the UAB community (for installation on UAB owned systems) and limited-use products based on a specific number of installations, users, or use.  The current agreement renews each August and requires an annual payment and an annual true-up (purchase of additional licenses) to cover additional FTEs over the base FTEs covered the previous year. 

The following Oracle products are covered under a site license and are available to the entire UAB system at no additional cost. They may be downloaded directly from Oracle by clicking on the product below. Server products are for use on Solaris, AIX, OS/390, UNIX, and Windows. Client products are for use on Windows and MAC.

LabView - National Instruments

Your new voice mail box has been created on the new system and we need you to setup your new voice mail box prior to the cutover on November 15th.

You will be able to access archived voice mail messages on your old voice mail until December 15th, 2012.  To access archived messages dial (6-6001) and follow the prompts.

How Do I Interpret My MAC Address?

 

Each ethernet device is assigned a unique hardware address by the manufacturer. Manufacturers are each assigned a range of addresses that they may use in their manufacturing, so that it is not possible for two vendors to assign the same address. These hardware addresses are called MAC Addresses.

SOLUTION:

You can search by MAC number to find out what vendor's hardware you are looking for. An interactive database is available from the IEEE for this purpose. You can also check Cavebear's list of Vendor Codes

WIFI Guest User Access

Connectivity

Industry standard 802.11 a/b/g wireless data network providing access to the campus network and Internet.  Access to the network requires authentications and authorization through the UAB enterprise directory.

Terms of service

All persons (including guests) utilizing the UABVisitor network are required to abide by relevant UAB policies, including the Policy for Acceptable Use of Computer and Network Resources (AUP). Guests must agree to the AUP when authenticating to the network. Any abuse of the service or UAB resources by a guest will result in termination of their access and potential suspension of the associated Allocation account where applicable. Additional setup or programming fees may be required for reactivation.

Privacy

Unencrypted sensitive data should not be transmitted through the UABVisitor network. SSL and/or VPN is strongly recommended for most uses. Guests should expect that their contact information and any records of network usage activity will be made available to legal authorities if requested through appropriate channels.

Security

Devices connected to the UABVisitor network should have all current OS security patches applied, and the use of personal firewall software is strongly recommended.

ID/Password

Each individual connecting to the UABVisitor network must be authenticated on a daily basis at minimum, with their own valid ID and password from the UAB enterprise directory. There is no option for shared, anonymous, or casual access.

SSID

In order to connect a device to the UABVisitor network, it is necessary to configure an SSID into its wireless adapter. The UABVisitor network is unencrypted and does not require an encryption key. Users are strongly encouraged to use SSL or VPN for sensitive data.

  

Target Audience

  • Plan A:  "Managed Allocation": Departments that routinely host short-term events such as continuing education classes, conferences, or sports camps. These events typically draw numbers of visitors to campus who request or require Internet access and can make use of Wifi capability.
  • Plan B:  "Ad-hoc Service": Departments that only occasionally host a visiting faculty, or vendor. These guests need Internet access temporarily and can utilize Wifi with their laptop in order make a presentation, contact company resources, or check their email.

Request Access and Provisioning

  • Plan A:  Guest Wifi Allocation account will be established which allows designated departmental staff to assign Wifi access for their visitors, through the use of an authenticated Web site at https://commservices.comm.uab.edu/guestwifi/login.aspx without ongoing requests to UABIT. The Allocation account will be provisioned with a maximum number of slots which can be in use at any given time.
  • Plan B:  Departmental finance officers and/or their designees can request Wifi access for a visitor on an as-needed basis, by submitting a service order to UABIT via an authenticated Web site at https://commservices.comm.uab.edu/guestwifi/login.aspx

Turnaround

  • Plan A:  Five business days to establish a new Allocation account, immediate guest access activation.
  • Plan B:  Immediate guest access activation

Cost

  • Plan A:  Setup fee of $20 per allocation account, and then $8.50 per month per available access slot
  • Plan B:  $1.50 per day per guest
