The UAB IT website has several query forms used to gather some basic information from our customers for a variety of purposes. In order for the IT Team to respond to these queries, you need to fill out these forms as completely as possible and, at the minimum, provide a valid email address.

This address is used for the sole purpose of responding to your queries via email. Your email address and other information may be stored in a tracking database so that we may monitor progress on your query, and will not be used for any other reason.
