New to the department? Filling out the form below will get you a CDIB username and password, which will allow you access to PCs, printers, and data stored on our file servers. Please be sure that you have a BlazerID before filling out this form. A "BlazerID" is a central username which will be your ticket to many UAB services throughout your tenure at UAB. It also provides you with an email address, which is This e-mail address is being protected from spambots. You need JavaScript enabled to view it . You can obtain a BlazerID at UAB's "BlazerID Central" webpage: http://www.uab.edu/blazerid. Be sure to click Yes to the question "Do you want a UAB email account?" Once you have your BlazerID, please print and fill out the form below and your Cellbio account will be created.
