Guidelines on submitting a protocol to the DOG HSR RC Each new protocol that is being submitted to the University of Alabama at Birmingham Institutional Review Board (UAB IRB) must include an approved Protocol Oversight Review Form (PORF) that indicates approval at the departmental level. For the UAB Department of Genetics, this PORF is issued by the DOG Human Subjects Research Review Committee (DOG HSR RC). Continued review of a protocol does not require review by DOG HSR RC or PORF. To submit to the Department of Genetics HSR RC, please provide all materials required by the UAB IRB (http://main.uab.edu/show.asp?durki=30246 ) as well as the one-page DOG HSR RC application form to the address listed below. After you submit, you should receive an e-mail acknowledgement when your information has been received. Please submit your protocol, DOG HSR RC application page, and other IRB required materials to the committee at least one week prior to our monthly scheduled meeting. The committee will review your protocol according to the calendar of regularly scheduled meetings. Response from the committee will be in the form of comments and/or completed PORF and will be received by the designated study personnel within five business days of the review meeting. Please indicate in the "Comments" section of the DOG HSR RC application form if there are special circumstances surrounding the regularly scheduled review calendar. After review, feedback or response via PORF will be provided to the designated study personnel within two days of the committee review meeting. |
