Parking
The closest parking is in the 4th Avenue Parking Deck (enter from 4th Avenue South between 18th and 19th Streets South). This is the primary public parking deck for University Hospital and connects to the North Pavilion via 2nd Floor Bridge. Once on the 2nd floor, follow concourse A (red) to concourse B (green), and turn right. At the dead end, turn left. You will then be at the Sparks Center. Take elevator to the 6th floor, the waiting room is right in front of the elevators (Room 600A). Valet, wheelchairs and wheelchair escort service are available on the 2nd level of this deck. If you need a wheelchair it is a good idea to arrive about 20 minutes earlier than your appointment. You can also ride on the golf cart that will get you close to the Sparks 2nd floor entrance.

Nature of Evaluation
The examination will consist primarily of interviews with professional staff, and pencil and paper tests. The evaluation may require the majority of the day, depending on the reasons that your doctor has for sending you to us. Please be sure to get a restful night's sleep and to eat a good breakfast before coming to the clinic. If you will be traveling a long distance to our office, it may be necessary that you make arrangements to stay in Birmingham the night prior to your appointment so that you will be well rested. In most cases, it will be necessary for the clinician to speak with a family member or friend on the day of your appointment. If possible, please make arrangements for this individual to be available in our office during the morning. Should your family member or friend not be available during this time, please contact our office so alternate interview arrangements can be made. Please be sure to take any medications that you normally would before coming to your appointment. Be sure to bring your glasses and/or hearing aids with you to your appointment. A lunch break will be taken for an hour at noon, and a cafeteria is available in our building for you to purchase your lunch.

Payment and Insurance
Medicare: We accept Medicare Part B for payment for these procedures, so you will not need to be concerned with insurance payment. If your insurance has changed, please call our office. Non-Medicare: You will be responsible for full payment of your bill. It is very important that you contact your insurance company to learn about your benefits. Our staff will be happy to assist you in filing any insurance claims. Because some insurance companies pay only a portion of the cost, we may request that you bring a partial payment with you at the time of the evaluation. In situations of this kind, we would arrange a payment plan to meet your financial needs. If a financial plan is needed it is important that this be arranged prior to your appointment.

Cancellation Policy
If you are unable to keep your scheduled appointment, please call us at least 36 hours before the scheduled appointment. Otherwise you could be billed a “no show” fee. Due to the nature of these evaluations, we can only schedule one or two patients a day. It is important that you notify us as soon as possible if you need to cancel or reschedule your appointment.

Patient Information Sheet
You should receive a Patient information Sheet in the mail. Please complete this form and bring it with you to your appointment. This information is very important to the psychologist and will be used during your visit.
