Scheduling an Appointment

A physician's referral is required to see a neurologist at UAB (patient must be at least age 17). The physician can be your family doctor or another neurologist or specialist who has recommended that you see a UAB Neurologist. THE APPOINTMENT SCHEDULING AND REFERRALS TELEPHONE NUMBER FOR ALL DIVISIONS OF NEUROLOGY IS: 205.801.8986. The UAB Neurology outpatient clinic is located on the 5th floor of The Kirklin Clinic located at 2000 6th Avenue South.

If you are not sure you need to see a neurologist OR need help determining which of our specialists you should see, you may also call UAB HealthFinder at 205.934.9999 or toll free at 800.UAB.8816 (800.822.8816). Depending on your medical problems, you may be directed to a neurologist who specializes in Nerve or Muscle disorders, Epilepsy, Headache, Parkinson's disease, Stroke, Alzheimer's disease, or other specific areas of neurology.

After Hours and Emergencies

If you need to reach your physician after hours, please call the UAB Department of Neurology Answering Service at 205.934.5690. If you do not know your physician's name, call UAB Paging at 205.934.3411 and ask for the Neurology Resident on call. If this is a true medical emergency, please dial 911 for immediate assistance.

What to Bring

Once you have been given an appointment to come and see us in our office, it will be our pleasure to provide you with the best neurologic consultation possible. However, in order for us to do this, we must have all pertinent information about your medical history. In general, the following items are of extreme importance in our ability to render an opinion about your case:

  • Hospitalization records pertinent to the condition that brings you for consultation
  • Reports of tests performed in relation to the condition that brings you for consultation
  • Films (x-ray pictures) of all tests performed in relationship to your condition, including CAT scans, MRI scans, angiograms, EKG, EEG, and ultrasounds (bring actual MRI films and EEG paper samples or CD if possible)
  • Your referring physician's outpatient medical records


Since we do not want to waste your visit to our clinic, we suggest that you personally secure the documents outlined above and bring them with you to your clinic visit. This will assure that we are able to discuss the information with you and that we can expedite your outpatient care. Do not mail or bring any material prior to the day of your visit. This increases the likelihood of things getting misplaced.

Also, please call to confirm your appointment 4 to 5 working days prior to the appointment . If you are local, please call 801.8986. If you are outside of Birmingham, please call 1.800.333.6543 and ask for the Neurology scheduling office. If you do not call to confirm, your appointment will likely be given to another patient. If the appointment time is given to another patient, you will be seen by the physician only if time permits. If you have to cancel or reschedule your appointment for any reason, please try to let us know 24 hours in advance.

What to Expect

As a reminder, our front-desk staff will be verifying our record information and may ask to see your insurance card. If your insurance requires a physician referral and/or precertification, the front-desk clerk will need this information. You must be able to provide it at the time of your appointment. If there is not a current referral and/or precertification, you will be responsible for your bill. You can confirm with your referring physician or your insurance company that appropriate referrals have been completed. Co-payments will be collected at our checkout desk.

Another point we would like to emphasize is that, in addition to the clinic, we also are responsible for the care of the patients admitted to The University of Alabama Hospital. Because of the medical emergencies related to this work, conflicts in the schedule and unexpected clinic cancellations may occur. We suggest that you call if you live outside the city of Birmingham on the morning of your appointment (before you get on the road) to assure that no emergencies have resulted in our clinic.
