The UAB Division of Neurosurgery serves a pivotal and critical role in support of comprehensive patient care provided by the UAB Health System. Our highly trained neurosurgeons in every sub-specialty field form the foundation of diagnostic and treatment services available to patients throughout the country.
Appointments / Referrals
A physician's referral is required to see a neurosurgeon at UAB. The physician can be your family doctor or another neurologist or specialist who has recommended that you see a UAB Neurosurgeon.

What to Bring, or Have Sent, Prior to Your First Visit
Once you have been given an appointment to come to one of our clinics, it will be our pleasure to provide you with the best consultation possible. However, in order for us to do this, we must have all pertinent information about your medical history. In general, the following items are of extreme importance in our ability to render an opinion about your case:
- Hospitalization records pertinent to the condition that brings you for consultation.
- Reports of tests performed in relation to the condition that brings you for consultation.
- Films (x-ray pictures) of all tests performed in relationship to your condition, including CAT scans, MRI scans, angiograms, EKG, EEG, and ultrasounds.
- Your referring physician's outpatient medical records.

(Insurance and demographic information (address, phone #, etc.) will be confirmed at each visit) - If Running Late, please call us to see if you can be seen or if you will need to reschedule.
What to Expect
As a reminder, our front-desk staff will be verifying our record information and may ask to see your insurance card. If your insurance requires a physician referral and/or precertification, the front-desk clerk will need this information. You must be able to provide it at the time of your appointment. If there is not a current referral and/or precertification, you will be responsible for your bill. You can confirm with your referring physician or your insurance company that appropriate referrals have been completed. Co-payments will be collected at our checkout desk.
Another point we would like to emphasize is , in addition to the clinic, we also are responsible for the care of the patients admitted to The University of Alabama Hospital. Because of the medical emergencies related to this work, conflicts in the schedule and unexpected clinic cancellations may occur. We suggest you call if you live outside the city of Birmingham on the morning of your appointment (before you get on the road) to assure that no emergencies have resulted in our clinic.

Cancellations
Please make every effort to keep your scheduled appointment. If you are unable to do so, it may result in a delay in our ability to see you. If you need to cancel an appointment, please call as soon as you know you are unable to keep the appointment so we can offer your appointment to another patient. Cancellations may call the assistant for their surgeon. See list below.

After Hours and Emergencies
If you need to reach a physician after hours, please call UAB Paging at 205.934.3411 and ask for the Neurosurgery Resident on call. If this is a true medical emergency, please dial 911 for immediate assistance.
Payment Information
Some insurance providers may require a referral or pre-certification before seeing a specialist of this nature. Please be sure to bring a copy of this referral or pre-certification with you for insurance/billing purposes. If you are unsure of what your insurance company requires, please contact them in advance of your appointment and they will be happy to answer any of your questions. Your co-pay is expected at the time of your visit; however, our financial counselors are available to discuss any problems or special circumstances. Our financial counselors may be reached Monday-Friday, 8:00 a.m. to 5:00 p.m. at (205) 801-9910. If your visit is being covered by workers compensation, please bring the name and phone number of your employer and case manager.
