How Do I Enroll?

The UAB OH&S Employee Health Enrollment Form is the start of this process for the eligible employee. The Occupational Health Professional reviews the medical history and work exposures in order to determine what services to provide the employee to ensure a safe and healthy work environment.

Biennial follow up is necessary to keep up to date on changing work environments. Annual updates of certain information (forms) may be required.

To Enroll:
  • Download the Enrollment Form, if you are enrolling for the first time.
  • Print and complete the form.
  • Send the completed form in a confidential envelope to:

  1. Your enrollment form will be reviewed by Occupational Health Professionals.
  2. After your form has been received in our office, please allow 1 (one) week for processing. If the form is incomplete or there are questions to be answered, this could prolong the process.
  3. You may be contacted if further clarification is deemed necessary.
  4. Once your form has been deemed complete, you are then considered to be enrolled and/or updated in the program.
  5. For your convenience, once successfully enrolled, emails for anything required will be sent two weeks prior to, the day of, and two weeks past the due date.
To Update Enrollment:
  • If your job-related exposures change or you are due for biennial reevaluation, download the Enrollment Renewal Form. If you are due for an annual form update, download the applicable form (Allergy Form, Annual TB Form, Latex Allergy Form) from the list at the right-hand side of this page.
  • Print and complete the form.
  • Send the completed form in a confidential envelope to:
