An evaluative counseling session should be held and performance appraisal forms should be completed at the end of an employee's initial six month probationary period and annually thereafter. The purposes of the performance evaluation program are to provide an opportunity for discussing goals for improved performance and to create a record about performance which may be considered in promotions, transfers, or other personnel decisions.

Copies of performance appraisals for Campus employees (non-hospital) should be sent to Records Administration in the Department of Human Resources for processing.

Copies of performance evaluations for UAB Hospital employees should be submitted to Hospital Human Resources.
