Disclosing External/Internal Activities and Financial Interests

UAB has revised and updated the electronic forms used to implement the new UAB Enterprise Conflict of Interest and Conflict of Commitment Policy.  These forms have been combined into one, easy-to-use electronic questionnaire called the "External/Internal Activity Approval/Financial Interest Disclosure".

UAB Investigators - that is, individuals, regardless of their title, position, or employment status, who are responsible for the design, conduct, or reporting of research as defined in the new policy - must ensure that information on file for their external activities and financial interests is current and accurate.
