This report provides a summary of your contact information currently recorded in EHSA. It also provides a list of the permits to which you are associated.

  • From the EHSA home page, click on the "Reports" icon.
  • Click on the report titled "My Personal Information" and a PDF will be generated.

If any of this information is incorrect, please send an email to ors@ors.uab.edu and indicate which information needs to be updated.

 

  • You may access your training history and print certificates here.
    • If the link does not work, be sure you are connected to the UAB on-campus network or VPN.
  • To print a certificate:
    1. Single-click the training history for which you want to print a certificate.
    2. Click on the "Training History Reports" button and select "Training Completion Certificate" for either 'RAM, X-Ray, Laser' or for all other EHS courses.
    3. A PDF will be generated that you can download or print.
  • To print your training transcript, Click on the "Training History Reports" button and select "Training History Individual".
  • To submit your Laser Registration, Form LR-1 online

    1. Login to EHSA via https://uab.edu/ehs/ehsa/ using your normal UAB username and password. If the link does not work, be sure you are connected to the UAB on-campus network or VPN.
    2. Click on the "Equipment" icon.

    3. Click on the "Laser Registration" button link shown below.

    4. Click "+Add"
    5. Fill out all of the fields you can in the "Laser Registration Information", "Service Information", "Laser Registration Properties", and "Custodians" sections.  Required fields are designated by a red asterisk (*).  If you aren't sure of the information for a required field, simply type "Unknown", with the exception of the "Serial #" field.
      • If no serial number is present on the laser, create one with the format PICodeNOSN01, where 01 is incremented to 02, 03, etc.
    6. In the "Laser Registration Properties" section, select, "Continuous", "Q-Switch", or "Pulsed" then click "+Add" directly underneath as shown in the image here to enter information like wavelength, beam diameter, etc.

    7. In the "Custodians" section, check the box next to "Search By PI/Permit" and select the faculty/staff Laser Supervisor's name from the PI drop-down menu.  Click the "Attach" button next to the Laser Supervisor's name as shown below.

    8. Scroll to the bottom of the page and click "Save".
    9. When you save your entry, the LSO will receive an email notification that your laser registration is pending approval.  The LSO will review the entry and when it is accepted, you will receive an auto-generated email indicating the laser is now recorded on the Laser Supervisor's laser inventory in EHSA.

This form is required for everyone to receive training credit even if there is no immediate intent to operate a device. 

In general, this should be done by the program coordinator or whoever works closest to the person.

  1. Go to passport.uab.edu and log in
  2. At the bottom of the left side menu, click on "Manage Guests" under "Sponsored Guests"
  3. Click on "Create a Guest Account"
  4. Enter the guest's information.
  5. If you're adding a group of people for a program or event, you can create a group to keep the accounts organized.
  6. If the person also requires LAWN (wireless internet) access, check the box under the "Services" section. Most people won't need this, just basic authentication.
  7. You can grant access for up to 1 year, but choose an appropriate expiration date. If the person will need access longer than 1 year, you'll be able to renew their access.
  8. You can either have them set their own password, or you can choose it and tell them.
  9. Add a message if you want which will go in the email to them.
  10. Click the checkbox agreeing to the conditions, then click "Save".

If you are setting up a guest account so that someone can log in and take training, they should have access in an hour or so.
