You are invited to submit a description of your community palliative care program or innovation. Share with colleagues how you are bringing palliative care to your community through programs, courses, innovations, or evaluation strategies that reinforce the Summit theme of Culture, Communication, and Care in the American South.

Submission categories include, but are not limited to, the following:

    -    Rural/community-based palliative care programs
    -    Hospital programs
    -    Hospice programs
    -    Quality improvement innovations
    -    Other programmatic posters

Information to be included:

    -    Poster title
    -    Presenter name(s) & role
    -    Program name, location, city & state
    -    A 300-word maximum word description of your program or innovation
    -    Maximum poster size is 42” x 56”

The topical focus of the poster session will be on practice, education, research and palliative care programs.
