Appointment Information

New Patient Appointments

All new patient appointments get scheduled through the Patient Access Center. A physician makes a referral, by filling out the Referral Form below and faxing it to 205.638.9919, along with a Medicaid Referral (if this applies). Prior to scheduling an appointment our GI Dedicated Scheduler will contact the parent first to schedule their appointment. All appointments are made for the first available opening. Once an appointment is made, The Patient Access Center will mail out a NEW patient pack three weeks prior to your scheduled date. Included in this packet will be an appointment letter, our No Show Policy, Pre-registration instruction card and a campus booklet with directions. If for any reason after your appointment has been scheduled you are unable to keep your scheduled appointment just give us a call at 205.638.5457 to reschedule your appointment.
