WELLSCREENS appointments are available in seven locations March 12-April 24.Register online now for the annual Wellscreens initiative, which begins March 12 and continues through April 24 in university and hospital locations across campus. Walk-ins will no longer be accepted this year, so book your appointment early for the greatest convenience.
The 15-minute confidential health screening, valued at $30 per person, is provided free from UAB Employee Wellness to all benefit-eligible UAB, UAB Hospital, UAB LLC, UAB Health System and UAHSF employees. It measures blood pressure, total cholesterol, LDL cholesterol, HDL cholesterol, blood sugar, BMI, triglycerides and cholesterol ratio/cardiac risk, giving participants a snapshot of their health status.
Wellscreens appointments are available at seven onsite locations:
- Campus Recreation Center, March 12-13
- Administration Building, March 14-15
- UAB Hospital West Pavilion, March 20-21
- The Kirklin Clinic, March 25-26
- UAB Hospital North Pavilion, April 2-4
- School of Nursing, April 16-17
- UAB Avondale, April 23-24
Employees eligible for My Health Rewards can earn up to $350 a year by accumulating points to reach tiers valued at $50, $150 and $350. To rise to the next tier, employees must complete key behaviors, which include actions such as age/gender specific screenings, care-management programs, teeth cleanings and a Wellscreen visit.
Unable to schedule an onsite Wellscreen visit now? Additional Wellscreens dates will be available in summer and fall. Also, employees can have their health care provider complete and return a Wellscreens Physician Fax Form by Dec. 31 to be eligible for the My Health Rewards incentive.
