To ensure a safe and healthy campus environment, all students must satisfy immunization/TB requirements to be medically cleared for enrollment at UAB. Students attending UAB must satisfy these requirements prior to the due date or risk having registration cancelled or a registration hold placed on the student account. Students must be compliant prior to enrolling or they will not be allowed to begin classes.

Records received will be processed and reviewed by UAB SHS Medical Clearance and this will be updated on your profile within (5-7) business days.

Information for current Non-compliant registered students with an IM HOLD.
You have an immunization hold because you have not yet satisfied your requirements. All students must satisfy medical clearance requirements, including immunization mandates, to be medically cleared for enrollment at UAB. This hold will prevent you from registering, making changes to your registration, such as adding/dropping classes, or withdrawing from a class for Summer/Fall 2023. Once you have complied with your immunization requirements the hold will be lifted from your account.
It is strongly recommended that you complete the requirements and upload your documentation immediately to UAB Student Health Services (SHS) through the Patient Portal, as outlined below, ensuring SHS has adequate time to process that documentation and remove your Hold. This is easily accomplished by taking a picture of the documentation with a smartphone and uploading it to your Patient Portal. Submit your immunization records via the Patient Portal.

How do I know if I have met my requirements?
To check the status of your immunization requirements you can log into to your Patient Portal and view your status under the “Medical Clearances” tab. All students will receive a secure message via the Patient Portal once any submitted documentation has been reviewed. This message will give you a status update on your immunization requirements.
If your immunization status on your Blazer Net account is listed as “Exempt”, this means you are currently registered for all on-campus classes and not required to complete the immunization requirements.
**Please note that with any change in your registration, your immunization status may change within 24 hours of the last change made. Please check your immunization status after making any changes to your registration.

How do I schedule an appointment for vaccinations and TB testing?
Students may receive vaccinations and TB testing at the UAB Student Health and Wellness Center. Please schedule an appointment using the “Patient Portal” button on your BlazerNET home page or call 205-934-3580. View Immunizations & TB Tests tab for details and costs.

Can I request an exemption?
Nonclinical students taking only online courses (distance learning), with no on-campus classes, are exempt from the immunization/TB requirements.
Students may request a medical exemption by following the instructions on the Exemptions page.

Are scanners available on campus?
Scanners are available in the following locations during their business hours:
- Near the reference desk at Sterne Library (flash drive necessary to save your scanned documents)
- Near the HUB desk on the first floor of Lister Hill Library (flash drive necessary to save your scanned documents)

- One Stop Student Services where staff can assist you

What should I do if I cannot find record of my immunizations?
If you are an Alabama resident, contact the medical clearance department via the Patient Portal and request assistance with reviewing the state registry for your immunizations. If your Primary Care physician used the Alabama state registry and submitted your blue card there, we can retrieve that information for you.
If you are not an Alabama resident, contact your local Health Department for assistance.
If you would prefer be tested for your immunity, there are some vaccines that Titers can be drawn for. For assistance with this, please contact the Medical Clearance department via the Patient Portal.
The following information applies to students who began attending UAB prior to Summer Semester 2016.

Students currently attending UAB must satisfy these requirements or risk having a registration hold placed on the student account.
*If you are in Nursing, Dentistry, Medicine, Optometry, Health Professions, Public Health or Graduate Biomedical Sciences:
You will not receive an evite. Instead, your school/program will contact you with instructions regarding your medical clearance process, how to create your CertifiedProfile and your deadline.

What are my requirements?
1a. Based on your program of study and citizenship, select the appropriate form below to be completed by your healthcare provider.

1b. If you already have copies of your original immunization records, you may submit those instead of the form provided above. In addition to these immunization records, you must complete and submit this health history form. Alabama residents should note that the state immunization "blue form" does not satisfy all UAB medical clearance requirements.
2. If you have a history of a positive TB test, the UAB TB questionnaire is required.

How do I submit my medical clearance documents?
You will receive an evite to your UAB email from CertifiedProfile inviting you to create an account and submit your immunization documents electronically. If you have not created your account, please click “Begin Order” in your evite, agree to the terms/conditions on the next screen, and click “Continue”.
If you cannot find your evite and need to re-issue it, please log into BlazerNET, click “Immunization Status” and click “Re-Issue Evite” in the Immunization section OR contact One Stop Student Services. If you have already created an account, please return to www.CertifiedProfile.com and log in using your email address as the username. Enter your password and click “Login” to upload and submit your documents.

How do I know if I have met my requirements?
To view your requirement status, please log in to BlazerNET and click "Immunization Status" on your student profile page. If your status is “satisfied” you have met your immunization requirements. If you recently satisfied these requirements, please allow time for the review of your documentation and update to your immunization status in BlazerNET. If your status is “not satisfied” log in to your CertifiedProfile to view the requirements you have not yet satisfied.

Can I request an exemption?
If any of your courses meet on campus, or if your program has the option to meet on campus, you are not exempt from satisfying medical clearance requirements.
Students in the following online-only programs are exempt from the UAB immunization policy.
