Certain categories of students are required to have major medical insurance (BC/BS, Aetna, VIVA, etc.) to cover hospital, emergency room, specialty care referrals, and lab/X-ray costs—grad students in the schools of Medicine, Dentistry, Public Health, Optometry, Nursing, and Health Professions; professional students; and all international students.

The best and easiest way to provide proof of your immunizations is to download UAB’s form and have your physician’s office fill it out. If you use our form, you won’t need to send other documentation. If you don’t use our form, you’ll need to have your State of Alabama Form IMM-90 (the “blue card”), chart records, and tuberculosis testing records sent to our office.
