This policy details available accidental death and dismemberment insurance policies available to UAB employees. This document includes schedule of payments and information on premiums.

This policy explains the process of collecting benefits upon the death of an active employee, an employee on leave of absence, a retired employee. Copies of death certificates of spouses or dependents should be turned in to the Human Resources Benefits Office. Deaths of a person named as a primary or contingent beneificiary on a UAB employee's policy, should be reported to the Benefits Office.
