The OSP has compiled this list to provide a centralized location on our website for UAB forms used in the proposal process, for sponsored clinical study submissions requiring clinical billing review, as well as the administration and management of awards. The completion and maintenance of forms play an important role in compliance issues and ensures that actions or activities which require approval(s) have the appropriate signature(s) to indicate that approval. Forms also ensure that UAB internal guidelines and procedures are followed and provide an audit trail should the need arise. Of course most sponsors have their own standard forms to use, such as pre-formatted budget forms and standard close-out forms. These will normally be referenced in their solicitation or policy guide and are available on their respective websites. If you have any questions regarding the use of any particular form, please contact your UAB Sponsored Programs Officer for guidance. Attention! Download the NEW UAB Extramural Support Checklist! You may need to update your Adobe Reader; it is free. Effective April 23, 2012 the Project Personnel List is absolutely required with certain submission types (Original/New, Competing Continuation, Change in PI, Transfer In and Resubmission), and must be included with the UAB Extramural Support Checklist when submitted to OSP. Also please note the regulatory approvals section, e.g., IRB, IACUC, OH&S of the checklist, has been moved to the Project Personnel List. Should any submission type affect the approvals section, you will need to submit the Project Personnel List and include any changes to the approvals section. Instructions for Filling in Acrobat FormsOn the UAB internal forms and the PHS 398 remote forms, you can type in the information on your browser, print the form by using the Acrobat print command, and then fill in the remaining blanks (e.g., signatures and dates). |
