Community Diabetes Awareness and Coalition Building
The Tennessee-Meharry Medical College joined the East Nashville community with community and faith-based organizations to form coalitions that address diabetes, obesity and other chronic illnesses. This project began in 2014 as part of the Mid-South TCC expansion to three additional southern states. One of the greatest needs has been for community stakeholders to get involved with combating chronic diseases, such as diabetes. Not only do these stakeholders know their communities, they are invested in their communities. If communities take ownership, there's a greater chance of sustaining healthcare efforts. Through the Mid-South TCC, these impactful stakeholders were identified through diabetes awareness events, classes, and health screenings. They were able to identify key people who want to be engaged to solve the problem.
