All Volunteers and Interns must complete a Volunteer Application Packet. If you are applying to volunteer or intern in prevention and testing, return your packet to Kelly Ross-Davis. You may email (complete and scan documents), in person (place in bin outside Room 188), or by mail. (See contact information below.) If you are applying for a Social Work internship, please return your packet to Kathy Gaddis, Coordinator of Social Services. Forms and Resources:• Volunteer General Packet (MS Word format) • HIPAA Training Module - use this to complete the HIPAA Quiz in the packet (PDF format) • 1917 Database Form (PDF format) and Confidentiality Agreement Form (PDF format) Please be sure to complete ALL paperwork. Note: If you have completed HIPAA training at UAB, please include your certificate or other evidence of training (available on your Blazer Net) in place of the HIPAA Quiz.
