The Civitan-Sparks Clinics takes referrals from family physicians, pediatricians, other health care providers, schools and family members. We accept many types of health insurance and Medicaid. If you have not received a referral from a health care provider you may need to get preauthorization from the health insurance company or the primary medical provider.
The Civitan-Sparks Clinics is a member of the National Health Service Corps. As such, we promise to serve all patients, offer discounted fees for patients who quality, not deny services based on a person's race, color, sex, national origin, disability, religion, sexual orientation, or inability to pay, and we accept insurance including Medicaid, Medicare and Children's Health Insurance Program (CHIP).
If your child is uninsured, contact us if you have questions regarding fees and payment. You may fill out the Patient/Family Financial Assistance Application to receive financial help.
