DATA SUBMISSION GUIDELINES
Please mail completed original forms to the Data Collection and Analysis Center (DCAC) at UAB. Handwritten faxed forms are not acceptable since they are often unreadable. Participating institutions are responsible for the organized storage of retained copies. Please use black ink to complete the PHTS forms-NOT a pencil.
Data is collected on a quarterly basis. Please submit all new listings, transplants, follow up and related events (rejection, infection, etc.). The first quarter due date is April 30th (all events January, February and March). The second quarter due date is July 31st (all events April, May and June). The third quarter due date is October 31st (all events July, August and September). The fourth quarter due date is January 31st of the next year (all events October November and December). Institutions who prefer submitting data as the events occur are welcome to do so.
After forms are submitted and received, the DCC will enter each form and contact you as they see questions or data entry issues. We will contact you via email and at each meeting with data issues. Glaring issues (i.e. missing identification, gender, etc.) will generate an immediate email query. Other issues that are not as important (i.e. weight/height discrepancies, missing lab data, etc) but still vital for proper analysis will be sent on a monthly basis.
