Forms will be distributed at orientation and additional forms are available from the Division of Predoctoral Education office (CHSB-20, room 376). Students must have the mid-block evaluation form completed by a preceptor who has had adequate contact with the student during the first two weeks of the clerkship. The form must be returned to the clerkship office at the end of the rotation. The mid-clerkship evaluation will not be a part of the students grade.

Physicians and residents will evaluate the students clinical performance according to the form which follows. The letter grade will be converted into a numerical grade.
