Electronic Request Forms for Services
The following forms are available:
- Classroom Reservation
Request a time slot in the LHL Electronic Classroom. (Time slot is not reserved until you receive confirmation from library staff.) Please use Education Form below to request a session on library skills or resources.
- Comments
Send your comments, good or bad to our Director, T. Scott Plutchak.
- Document Delivery*
Request photocopy and document delivery service for journal articles that Lister Hill Library owns (using the same forms as ILL). Available to UAB faculty, staff, and students.
- HIP Patron Enrollment
Want to access your Lister Hill Library account 24/7 to renew your items, request a hold for a book, or just to see how many items you have checked-out and when they’re due?
- Interlibrary Loan (ILL)*
Request that LHL borrow a book, book chapter, or journal article from another library for your use. Service available to UAB faculty, staff, and students.
- Patron Registration Form (PDF)
Library patrons can print out and submit this signed form to be sure they are in the library database (for checking out materials and scheduling the classroom).
- Reserve Form
Instructors may request that library materials (or their personal items) be placed on Reserve for use by their entire class.
