Purpose
Physical Therapist is responsible for conducting initial and follow-up assessments/evaluations for adult consumers/participants. Additionally, PT will prepare required reports/findings, participate in conferences as needed and coordinate/lead/conduct group and individual treatment sessions and consult with parents and caregivers as needed. Other duties, as assigned.

Education / Experience
Minimum of two years experience in PT is preferred; however, new graduates will be considered
Graduated from an accredited program in Physical Therapy
Registered and licensed (Alabama Physical Therapist), or eligible for registration and licensure and will perform all duties under the supervision of a registered and licensed PT until licensure is obtained.

POSITION SUMMARY
Exempt (Salaried - Not eligible for overtime) To evaluate, plan and administer medically prescribed physical therapy treatments to assist patients inrehabilitation and in reaching their maximum physical capabilities.

MINIMUM REQUIREMENTS
Work experience may NOT substitute for minimum education requirement. Bachelor's degree in Physical Therapy.

•Plans and provides treatment to assigned patients as appropriate. Implements timelydischarge from treatment including patient/family education, creating a home trainingprogram and referrals for follow-up services and equipment as necessary.
•Acts as clinical instructor or mentor for Physical Therapy and Physical TherapyAssistants, students and new employees.
•Completes evaluations and assessments of assigned patients in accordance with theestablished guidelines and standards of care and practice.
•Participates in departmental in-services, new programs and quality improvementactivities that promote Physical Therapy, safety and staff development.
•Completes stat sheets and charge slips for each patient prior to the end of the shift.Submits equipment charges for all patients that received billable equipment.
