Physical Therapist is responsible for conducting initial and follow-up assessments/evaluations for adult consumers/participants. •Plans and provides treatment to assigned patients as appropriate. Implements timelydischarge from treatment including patient/family education, creating a home trainingprogram and referrals for follow-up services and equipment as necessary.
•Acts as clinical instructor or mentor for Physical Therapy and Physical TherapyAssistants, students and new employees.
•Completes evaluations and assessments of assigned patients in accordance with theestablished guidelines and standards of care and practice.
•Participates in departmental in-services, new programs and quality improvementactivities that promote Physical Therapy, safety and staff development.
•Completes stat sheets and charge slips for each patient prior to the end of the shift.Submits equipment charges for all patients that received billable equipment.
