Late Appointments
If you are running late, please call the clinic. If you are more than 20 minutes late and have not notified the clinic, it may be necessary to reschedule your appointment for another time. The decision to reschedule will be made by your appointment provider.Missed Appointments
It is very important that you call and let the receptionist know when you are unable to keep a scheduled appointment. By letting the clinic know in advance that you cannot be present at the appointed time, the clinic can reschedule another patient in your place.If you miss three (3) consecutive appointments without notifying the clinic prior to the time of your visit, you will be required to talk to the Study Coordinator or Principal Investigator if currently enrolled in a study, or the clinic Community Engagement/Recruitment (CER) Program Coordinator if attempting to enter a study to make any future appointments.
Reinstatement will take place only after successfully resolving your situation to the satisfaction of the Study Coordinator, Principal Investigator, or the CER Program Coordinator.
What to Bring
You should bring a list of your current medications to each appointment. Currently, you are not required to provide a photo ID or Social Security card for identification purposes; however, if you do not know your Social Security number or if you have had a recent change of address, you may want to bring these documents with you to ensure that your records and compensatory information are up to date. If necessary, your nurse or recruiter may instruct you to bring additional documents with you.Be Prepared
If you have a consultation appointment, be prepared to read through and discuss an entire consent document and any related materials. These documents range anywhere from 5 to 30 pages in length. It is also recommended that you bring a list of questions that you would like for your recruiter to answer. Please make any important phone calls before your consultation begins and keep your phone on silent or vibrate throughout the duration of your appointment.If you have a study visit, be sure to bring any documents or study-related items (swab kits, fluid-collection kits, specimens, etc.) that your nurse instructed you to bring. On days you are having blood drawn, be sure to eat breakfast and drink plenty of water.
While children are allowed to remain with you during a consultation appointment, they will generally not be allowed back with you during study visits (only on a case-by-case basis). Older children may remain in the waiting area while you finish your appointment; however, clinic staff will not be responsible for watching them. It is not recommended that you leave children under 13 years old unsupervised. Please ensure that you have adequate child care before scheduling your appointment(s).
