An evaluative counseling session should be held and performance appraisal forms should be completed at the end of an employee's initial six month probationary period and during the annual performance evaluation cycle. The purposes of the performance evaluation program are to provide an opportunity for discussing goals for improved performance and to create a record about performance which may be considered in promotions, transfers, or other employment decisions.

Performance evaluations for UAB Hospital employees are to be completed and maintained in the UAB Hospital electronic evaluation system.
