Clinical Patient Forms

With the transition to Epic, most clinical patient forms are only needed during downtime events and are available on the Business Continuity Access (BCA) device. The need to routinely bulk print and scan clinical forms has been drastically reduced through Epic functionality.

All unique facility-specific documents are currently undergoing review to determine whether they should be standardized and published for enterprise-wide use.

Any requests for new or existing clinical patient forms must be reviewed and approved by the Office of Medical Record Governance.
