Short-Term Non-Clinical Students & Scholars (International)

The University of Alabama at Birmingham requires the following testing for short-term international students/Scholars. Students/Scholars enrolled in non-clinical activities for three months or less at UAB will be considered short-term.

Short-Term Non-Clinical International Immunization Form

Tuberculosis Screening Requirements

  • The purpose of screening these short-term students is to rule out active TB disease.
  • The screening requirements were formulated in conjunction with the Jefferson County Dept. of Health recommendations.
  • TB Questionnaire must be completed and submitted to UAB SHS prior to arrival on campus
    • Positive result on question #7, symptom screening, will require the student/scholar to schedule a visit with a Student Health provider and the Jefferson County Department of Health (JCDH) upon arrival to campus. The student/scholar will not be allowed on campus until cleared by the UAB Student Health Services and the JCDH.
  • A CXR is required for all international students/scholars, performed in the United States, within 4 weeks prior to arrival on campus
    • If positive, proof of initiation of appropriate therapy is required, and the student will not be allowed on campus until cleared by the Jefferson County Department of Health and the UAB Student Health Services.
  • • Students with a positive CXR, positive symptom screen on TB Questionnaire, or are currently being treated for TB, will require evaluation at the JCDH upon arrival to campus, followed by a visit with a Student Health provider, and will not be allowed on campus until cleared by the JCDH and UAB Student Health Services.
  • Students/Scholars with a negative CXR and TB Questionnaire, and appropriately cleared as above, are cleared regarding the TB requirements.

Immunizations

  • MMR (Measles, Mumps, Rubella)
    • Must have had 2 doses of measles-mumps-rubella containing vaccine (MMR) at least 28 days apart with the first dose given after 12 months of age.
    • If the student has never been immunized, two injections of the vaccine, at least 28 days apart, are required.
    • In lieu of being re-immunized, such individuals may present proof of having had such immunizations. Proof must be either official documentation or certificates of immunization or positive titers to measles, mumps, and rubella.
  • Tdap
    • Evidence of at least a single dose of Tdap for all students aged 11-64 is required.
    • In the absence of proof of immunization, Tdap can be administered regardless of interval since the last tetanus-diphtheria toxoid-containing vaccine.
    • Adults should receive decennial Td boosters, beginning 10 years after receiving Tdap.
  • Meningitis Vaccination
    • Students up to and including 21 years must provide proof of immunization against meningococcal meningitis (Menactra, Menveo, or Menomune) since turning 16 years of age even if a vaccine dose was given at an earlier age.
    • The booster dose can be administered any time after the 16th birthday.
  • Varicella Vaccination
    • Required for all without evidence of immunity.
      • Evidence of immunity is defined as:
        • Written documentation of 2 doses of varicella vaccine
        • A history of varicella disease or herpes zoster (shingles) based on healthcare provider diagnosis, requiring provider documentation.
        • Laboratory evidence of immunity (VZV IgG antibody titer)
    • Two doses of varicella containing-vaccine given at least 4 weeks apart are required
      • Required if the student does not meet any of the criteria above or have equivocal or negative VZV IgG antibody titers.
    • For those students with a documented history of only one prior dose of varicella vaccine, one additional dose is required, at least 4 weeks after the initial dose.

  • If individuals do not have proof or have not been immunized, they must be immunized against these diseases prior to attending, enrolling, or participation in UAB academic, research, observing, or clinical program activities.
  • Enrolling students must show proof of these immunizations with either an official certificate of immunization, a photocopy of an immunization certificate, or written documentation from their physician. Students must provide proof of their immunizations and TB testing. The preferred and most efficient method to accomplish this is to download the Short-Term Non-Clinical International Health History form. This should be completed by the documenting physician’s office and faxed, emailed, or mailed to UAB Student Health Services by the student. The actual "Blue Form," chart records, and TB records need not be sent if this form is properly completed. The student may send these records in lieu of the Immunization Record Form.
