Web-Based Medical Clearance

Only students who are mandatory health insurance students will utilize the web-based medical clearance process.

The Medical Clearance process will be accomplished through a web-based program as of 6/29/11. The service is provided by CertifiedBackground.com, a secure, confidential, web-based company that is HIPAA/FERPA compliant. They will provide an Immunization Tracker and Certified Record Review.

Students will receive an instruction sheet from their respective schools/programs. The student will log into CertifiedBackground.com and create an account by simply following the referenced instruction sheet and the prompts once logged in. Each School/Program will have a unique “package code” provided in the instruction sheet. When creating your account, you will be asked to provide your social security number: you should enter all zeros in this field as this is not essential information and need not be supplied.

Once your account is created, you will follow instructions and upload your immunization information, preferably using the UAB Student Health Immunization Form (available on this website) or actual physician documentation (blue forms, etc.). This information will be reviewed, and you will receive automatic emails for incomplete records or other problems. You will need to log into your tracker--after 72 hours to check your tracker’s progress--and periodically thereafter, until all requirements are approved. You will periodically receive automatic notices of required impending clearance updates (annual TB renewal, etc.) from CertifiedBackground.com. You will be able to access and view your records at any time by logging into your account. 

Student Health Services now operates using a two-tiered Student Health Fee, and this fee covers the cost of medical clearance through CertifiedBackground.com. Mandatory students are automatically charged this fee through their Banner account and need not do anything else to pay for this service.
