Because each patient’s dental needs are unique, we are unable to provide an accurate cost estimate for treatment until a patient is evaluated through an initial exam by one of our providers. During the initial exam, our providers will discuss treatment recommendations and outline approximate costs in a treatment plan. This process allows patients to be fully informed of their options and aids in financial planning. More information can be provided at the time of scheduling. Payment is due at the time of service.

Payment is due at the time of service. Payment can be made by cash, check, or major credit card (Visa, MasterCard, or Discover).
In-Network Insurance
The UAB Dentistry Dothan Clinic is a preferred provider organization for Blue Cross and Blue Shield of Alabama, MetLife group plans, United Concordia, and Southland Benefit Solutions and will file claims on behalf of the patient to these insurance carriers.
Out-of-Network Insurance
Patients who maintain dental insurance with companies other than our in-network insurance providers (listed above) will be required to pay the entire amount of their bill at the time of service. Our office will provide the forms for the patient to file a claim with their insurance company for possible reimbursement.
