Due to an increase of incomplete submissions, effective Monday, February 11, 2019, OSP will no longer accept submissions that include missing documents and/or unsigned or incomplete Extramural Checklists.    

Statement of Purpose

Extramural Checklists are required by UAB in order for OSP to accept and set up records in the system(s) and begin the review process for grants and contracts.  It is used entirely for internal review; it is never transmitted to a sponsor.  The form has several purposes:

• It provides basic information regarding the project for review by the department head and other administrators.
• It ensures that all compliance obligations have been identified, considered and met, or are pending.
• It provides the signature approvals, including Principal Investigator certification/assurances, necessary to submit an application for external funding.
• It contains important information for institutional databases which facilitate reporting on institutional, school, department, and faculty metrics.
