A de-identified (no HIPAA identifiers) dataset from UAB Study of Aging, Phase 1 will be provided upon receipt of the following:

1. A completed & signed Data Request form. Note that “Recipient” is the person requesting the data.
2. A copy of the Institutional Review Board (IRB) approval for the study for which the data is requested.

Note that the data use agreement sets forth the following conditions:

1. Data are to be used only for the described and approved research project
2. The agreement is non-transferable
3. No attempt will be made to identify individual study subjects

The UAB Study of Aging investigators have made a substantial commitment to establish and maintain this data base. Collaborative relationships with outside investigators are invited, and researchers are encouraged to contact the UAB Study of Aging investigators to determine if their proposed research overlaps with work already underway.
