  • Personal Leave


    Definition

    Personal leave is for any condition/situation that is outside of medical or military leave of absence (e.g., mission trip, disaster recovery, etc.), including leave for employee illness/injury and pregnancy when FMLA eligibility is not met or for family and medical reasons beyond FMLA limits.

    Employees should contact their Department Representative or Supervisor for Personal Leave options.

    • One-year continuous service may be required to take personal leave. Exceptions may apply for medical related conditions.
    • Personal leave requires prior approval of supervisor/manager. Expectations may apply for medical related conditions.
    • Personal leave requires written notice 30 calendar days prior to begin date.

    • Paid Living Donor Leave Eligibility Requirements
    • Regular part-time and Alternative Work Schedule employees will receive prorated paid time off days based on the percentage of normally scheduled work hours within a 40 hour week.
    • Medical related leaves will be approved only for the length of time your health-care provider certifies.

  • Military Leave


    Definition

    There are two categories of military leave:

    1. Military Leave for Employee (self)
    2. Military Family Leave
      • Qualifying Exigency: Eligible employee may take qualifying exigency leave when the employee's spouse, son, daughter or parent who is a member of the Armed forces (including Nat. Guard and Reserves) is on covered active duty or has impending call or order for active duty.
      • Military Caregiver: Eligible employee who is the spouse, son, daughter, parent or next of kin of a covered service member with a serious injury or illness.

      • Max 21 working days with pay per calendar year provided to all employees ordered to military duty.

  • Sabbatical

  • Educational Leave


    Definition

    An Educational Leave of Absence is available to regular staff employees for up to 12 months to pursue education or training that will lead to increased job ability.

    A formal, written request for educational leave must be submitted to the employee's department head at least one month before the beginning date of the leave. Educational leave is subject to the approval of the hospital administrator or appropriate dean or vice-president.
