UAB Medical Genomics Laboratory is contracted with most major insurance providers. Please view our list of participating insurance plans.
Information for Patients
- Please include a legible copy of the front and back of your insurance card.
- If you have questions regarding a billing statement, please contact the phone number listed on the statement.
- Rush testing fees are not billable to insurance and must be paid by the patient before testing.
- If your insurance provider is not included on our list of participating insurance plans, please contact your insurance company to determine your coverage and eligibility.
Information for Clinicians
- Please include a legible copy of the front and back of the patient's insurance card.
- UAB Medical Genomics Laboratory will contact the insurance provider to verify CPT code coverage for all specimens submitted for insurance billing. The ordering provider will be contacted if:
- Coverage for the requested CPT codes is denied.
- Additional clinical documentation is required to determine coverage.
- The patient's expected copay or deductible exceeds $500.
