Diversity, Equity and Inclusion Submissions Form

The UAB School of Optometry is committed to providing an inclusive environment where everyone has a voice. To that end, this form is intended to gather Diversity, Equity and Inclusion (DEI) concerns, recommendations and celebrations from UAB School of Optometry students, faculty and staff so that they can be heard.

Submissions are anonymous. However, if you would like to be contacted by the UABSO Director of DEI, Dr. Keisha Brown, please include your contact information below. Dr. Brown is the only person who will view submissions.
