UAB State of Slim Sign Up Form

Thank you for your interest in the State of Slim program! Our inaugural class will be offered free to a select group of participants. The selected participants in the first group will agree to have their entire journey shared publicly on the UAB and State of Slim websites and social media accounts. In order to have a balanced group of representatives, we unfortunately cannot choose everyone who applies. If you are not chosen for the first group, we will be in touch with you about participating in future groups.

There is a designated committee that will review all candidates. They will select the inaugural class based on many variables including, but not limited to, health condition. Anyone selected as a candidate, will go through an extensive review and interview process before a final decision is made. We hope to have our first class of candidates select by the end of summer or early fall.
