Since November 2012 a group of UAB medical students have provide basic primary care to residents in the Jefferson County Housing Authority Shelter Care Plus Program. EAB runs three clinics; chronic-care, acute-care and mental-health, Patients need a referral from either the acute-care or chronic-care clinic to be seen at the mental-health clinic.

Born of the UAB Nursing academic practice partnership between the school and hospital, this innovative, replicable, collaborative-practice model clinic comprises an interprofessional team of nursing, medicine, social work, health services administration and health information technology professionals. It targets the underinsured and medically underserved.

The clinic is funded through a grant from the Health Resources Services Administration (HRSA) and supported through an ongoing academic/practice partnership between the UAB School of Nursing and UAB Hospital.

Alabama ranks first in the nation in prevalence of diabetes. Many underserved or underinsured citizens in the Birmingham community suffer from this disease without access to care. The University of Alabama at Birmingham (UAB) School of Nursing Providing Access to Healthcare (PATH) Clinic, which has been providing high-quality care in an interprofessional clinical setting since 2012.

The PATH clinic, in partnership with UAB Medicine, serves patients recently discharged from UAB Hospital. It is located in the Cooper Green Mercy Health building at 1509 6th Ave South, Birmingham, AL 35233 – Suite 4400.

The PATH clinic employs a variety of health care professionals, including nurse practitioners, physicians, dieticians, optometrists and social workers to provide the complete spectrum of care for patients living with diabetes. In addition, students in each discipline have the opportunity to become involved through coursework or clinical placements.

The UAB School of Dentistry, in partnership with the schools of Medicine, Nursing, Health Professions and Optometry, received a $150,000 renewal grant through the DentaQuest Foundation in late 2012 to continue a pilot program designed to train health-care students to work collaboratively and recognize when they should make referrals to other professions.

“The project allows us to conduct free, school-based and childcare center-based health screenings for children in selected Birmingham schools and the surrounding area,” said the grant’s principal investigator, Conan Davis, D.M.D., director of community collaborations at the UAB School of Dentistry. “Students are the most impressionable while in their professional training. We think it’s important for them to learn how and when to collaborate, and when it’s necessary to refer, as well as to know what they are looking for on a very basic level. This will help all of the disciplines better serve their patients when they go out into the real world.”
