UAB Medical Clinic at the Foundry

The University of Alabama at Birmingham School of Nursing partners with The Foundry Rescue Mission and Recovery Center in Bessemer, Alabama to operate a nurse-managed clinic for both Foundry residents and community members in need of affordable health care. The clinic provides primary care and urgent care for health concerns that might otherwise go untreated, including diabetes, high blood pressure and heart disease, for patients ages 14 and older.

Hours of Operation

The UAB Medical Clinic at the Foundry is open 36 hours per week with clinic hours as follows:
Monday: 8:30 a.m. - 5:30 p.m.
Tuesday: 10:30 a.m. – 7:00 p.m.
Wednesday: 8:30 a.m. - 5:30 p.m.
Thursday: 10:30 a.m. – 7:00 p.m.
Friday: 8:30 a.m. - 12:30 p.m.


Appointments are recommended but walk-ins are welcomed.

HTRSA Heart Failure Clinic

Born from an academic-practice partnership, the Heart FailuRe Transitional Care Services for Adults (HRTSA) Clinic is a nurse-managed interprofessional collaborative clinic for underserved patients with a diagnosis of heart failure.

Mission: To provide guideline directed care and education to underserved patients with heart failure and their caregivers in an interprofessional collaborative practice across care transitions.

Transitional Care Model: The HRTSA Clinic provides services across the care continuum by seeing patients in the hospital, clinic, and home.

Referral: Patients must have a diagnosis of heart failure and be underserved.

PATH Clinic at UAB Medical Towers

2M4A4227Under the direction of Michele Talley, PhD, ACNP-BC, the University of Alabama at Birmingham School of Nursing partners with UAB Hospital and Health System to operate the nurse-managed PATH (Providing Access to Healthcare) Clinic in the Medical Towers Building at UAB. This collaborative effort, a continuation of the PATH Clinic formerly located at M-POWER Ministries, utilizes an interprofessional, team-based model to provide diabetes care at no cost to an uninsured medically underserved population discharged from UAB Hospital. Services provided include nurse practitioner visits, physician visits, optometry, nutrition, mental health, and assistance in obtaining needed medications at the lowest possible cost.

Hours of Operation

The PATH Clinic at UAB is open two days per week with clinic hours as follows:
Tuesday – 8:00 a.m. – 12:00 p.m. and 1:00 p.m. - 5:00 p.m.
Thursday – 8:00 a.m. – 12:00 p.m. and 1:00 p.m. - 5:00 p.m.


Diabetes patients being discharged from UAB hospital: Appointments are made by hospital social workers/case managers using the PATH referral form. The form will soon be located on the SCR website. In the interim, UAB Hospital social workers and others who wish to make a referral should call (205) 996-0153 for more information or to obtain a referral form.

Current patients: Call (205) 209-6010 for medical issues between 8:30 a.m. and 4:00 p.m. Monday through Friday. For scheduling issues, call (205) 996-0153. Parking is to the side of the buidling and is free. Street parking may also be available.
