What is a Wellscreen visit?

A Wellscreen is an onsite health screenings that take 15 minutes to complete. The screening is free, confidential, and includes a finger stick.



How do I register?

Visit uab.edu/wellscreens and click on "Register Online" to select the screening time that works best for you. If you have to miss your appointment, please be sure to cancel it online in order to open up the space for someone else.



How can I get my screening results?

Your Wellscreen results will be written down and given to you immediately following your appointment. Please allow 10 days for your results to appear on your My Health Rewards account, which you can reach by visiting uab.edu/myhealthrewards.



Do I need to bring my UAB ID badge?

Yes, your UAB ID is required to check in at the screening site.



Can I eat or drink anything before the screening (and be fasting)?

Yes, but you are encouraged to fast to improve the accuracy of the results.



Who is eligible to participate?

Wellscreen are for all UAB, UAB Hospital, UAB LLC and Health System/Health Services Foundation employees who are eligible for a medical plan.



Can I participate in an onsite Wellscreen appointment during work hours?

Yes, employees can participate in screenings during work hours, still subject to manager approval.



Do you take walk-in appointments?

In an effort to decrease wait time and make it even easier to get in and out, we’re NOT accepting walk-ins this year. Pre-schedule your appointment to ensure that you can be screened in the location at the day and time that works best for your schedule. Registration may book early but will not close, so check back online regularly to see if there has been a cancellation.



What metrics are included in the onsite health screening?

Your Wellscreen visit will measure blood pressure, total cholesterol, LDL cholesterol, HDL cholesterol, blood sugar, BMI, triglycerides and cholesterol/HDL ratio.



How is my confidentiality protected?

You may have some concerns about the confidentiality about your personal information. In compliance with HIPAA (Health Insurance Portability and Accountability Act), your personal information from a Wellscreen will NOT be shared on an individual level with your employer. UAB will receive aggregate information (without names) in order to design future wellness initiatives. View the Notice of Privacy Practices.



How does a Wellscreen appointment benefit me?

Knowing your personal numbers matters. Health screenings can identify unknown conditions, diseases, or risks for disease that can significantly influence the quality of one’s life for the future. A simple drop of blood can provide clues about how to improve one’s health and wellbeing. This is part of UAB’s ongoing commitment to support each employee’s health and wellbeing.



Who will have access to my personal data?

UAB HealthSmart will privately and securely release your Wellscreen data to Chip Rewards who will upload your information on your confidential My Health Rewards profile. UAB HealthSmart and Chip Rewards protect all screening data through secure processes and protocols to ensure confidentiality.



What are the credentials of the Wellscreen staff?

The staff consists of professionals who have been certified by UAB HealthSmart to conduct health screenings according to protocols and include registered and licensed practical nurses and health educators.



What other option do I have if I am unable to make a Wellscreen appointment during the campaign?

If you are unable to attend one of the screening events, you have the option to have the appropriate measures taken by your primary care physician using the Wellscreens Physician Fax Form. A Physician Fax Form must be completed by your health care provider and faxed to UAB HealthSmart (205-996-2974) by December 31, 2020, to receive completion credit for incentive (if applicable). No exceptions. Please confirm with your physician that your visit is a preventive exam visit to ensure that it is recorded (“coded”) appropriately.



How can I qualify for a reasonable alternative?

Participants who cannot meet the standards outlined by the Wellscreen program for medical reasons may qualify for a reasonable alternative and may still have an opportunity to qualify for the reward. Please contact UAB Employee Wellness to request a reasonable alternative at uabwellness@uab.edu.
