• UAB Vehicle Safety Management Program

The Vehicle Safety Management Program ("VSMP") provides overall guidance and explains the responsibilities for departments and drivers associated with operating a vehicle on UAB business. The VSMP also provides guidance related to the use of golf and utility carts at UAB.

• MVR Release and Request form (online version)

This online form may be used to initiate a request to review of an individual's driving record. The name of the supervisor or department administrator to whom results should be sent is required when completing this form.

• UAB Vehicle Accident Report Form

The form should be complete by the UAB driver as soon as possible following an accident. This is the driver's opportunity to record the events of the accident in his/her own words.
