  • RSForm!Pro

    The RSForm!Pro plugin allows Joomla web administrators to quickly and easily create online forms. The interface uses a grid system to allow form creators to drag and drop form fields.

    Features:
    Supports file uploads, such as images or documents
    Supports multi-page forms and dynamic content
    Ability to create customized emails to deliver submissions, including user confirmations
    Includes a logic component for building conditionals and calculations
    Fields have automatic validation rules that may be customized
    Forms are protected from spambots by reCaptcha technology
    Allows custom html, css, javascript and PHP scripting
    Ability to export submissions to Excel, XML, CSV, and PDF
    Forms can be duplicated to multiple sites

  • Qualtrics

    The Qualtrics Research Suite is a powerful online survey tool that allows you to build complex surveys that fulfill a variety of research needs. You can use this tool to build surveys, distribute surveys and analyze responses, all from one convenient online location.

    Features:
    Supports file uploads, such as images or documents
    Capable of complex conditional fields
    Supports multiple page forms and saves partial submissions for users to complete at a later time
    Includes advanced analytics
    IQ Score tool provides feedback to form creator based on accessibility, methodology and survey errors
    Can limit submissions by individuals, offer password protection, or limit access by http referrer verification
    Forms can be displayed on multiple sites
    Meets WCAG standards
    ISO 27001 certified and FedRamp Authorized for data security

  • FormAssembly

    FormAssembly is preferred for lead forms for student recruitment. Please email marketing@uab.edu for more information.

  • Formidable

    Formidable is a drag and drop form building solution provided on UAB WordPress sites.

    Features:
    Supports file uploads, such as images or documents. Upload field can repeat to allow multiple on-demand fields
    Includes a repeater option for users that may need to add additional fields to their submission
    Fields include basic validation rules
    Forms are protected from spambots by reCaptcha technology
    Provides basic analytic tools to analyze submissions
    Ability to export submissions to CSV or XML, max size 100MB
    Meets WCAG standards
