Education and Mentoring to BRing Access to CarE SCD Network
Project Rationale
EMBRACE is a HRSA-funded sickle cell disease treatment demonstration project regional collaborative of 8 southeastern states (AL, FL, GA, KY, MS, NC, SC, TN). We work together using collective impact strategies and continuous patient engagement to improve access to high-quality care for individuals living with sickle cell disease (SCD).
Project Goals

To increase the number of providers treating SCD according to the National Heart, Lung and Blood Evidence-Based Management of Sickle Cell Disease Expert Panel Report (NHLBI SCD Guidelines.

To enhance the use of virtual care models including: tele-mentoring, telemedicine, and other technologically-based provider support strategies to increase the number of providers adhering to evidence-based SCD care.

To develop and implement strategies to improve access to quality care emphasizing individual and family engagement and adolescent transition to adult care in a medical home.
We also provide extensive peer-education for other medical providers including physicians, advance-practice providers, nurses, social work, and others.
Project ECHO will connect local clinicians with sickle cell experts to learn skills to deliver high quality evidence based sickle cell disease care using this proven tele-monitoring model. The goals of this activity are to close the gap in provider knowledge and comfort with caring for sickle cell patients, increase the number of providers willing to provide quality evidence based care for sickle cell disease patients.
We participate in ECHO tele-mentoring with multiple sessions available every month (both through UAB as well as through our partners at Atrium Health)
