All students and employees, including faculty, staff and UAB Medicine employees, coming on site must complete UAB Healthcheck daily (7 days a week), a web-based COVID-19 assessment tool that allows users to report COVID-19 symptoms and exposure history daily. UAB Healthcheck consists of surveys that assess symptoms and exposure to COVID-19.

For quick and easy access, Healthcheck will now send a daily text message allowing you to respond via text to the three questions pertaining to symptoms, exposure and current quarantine status with a simple “yes” or “no.” Based on your response, you will receive further instructions via text message on next steps.

The new UAB GuideSafe Hotline — 205-934-SAFE (7233) — has been established to give employees and students a single source for answers to common COVID-19 questions about testing, exposure, Healthcheck, isolation, and more, including troubleshooting technical questions about the GuideSafe Exposure Notification App and UAB Healthcheck. 

Visit UAB Healthcheck DAILY to retrieve your Passport. Bookmark your Passport page. Your BlazerID will not be displayed for security reasons.

How UAB Healthcheck Works

Fill out the initial set up survey the first time you access UAB Healthcheck.

After that, you are required to complete UAB Healthcheck daily (7 days a week).

Access UAB Healthcheck directly via the UAB app or https://healthcheck.staysafetogether.org/14. Employees can also access UAB Healthcheck through the myUAB employee portal.

Set up email and text reminders through the UAB Healthcheck tool.

If you develop new symptoms — do not wait for a reminder — report them immediately. You cannot fill out Healthcheck twice in one day. If you have already filled out Healthcheck for that day, alert Student Health or Employee Health:

  • Faculty and staff should fill out the Employee Health Symptom and Exposure form online.

UAB Healthcheck: Required DAILY 7 Days a Week

Students and employees are required to complete the COVID-19 assessment tool, UAB Healthcheck on a daily basis (7 days a week), and their assigned risk level will automatically be communicated to the GuideSafe™ Event Passport system. Through Healthcheck, you report whether or not you have symptoms or exposure to anyone who has recently tested positive for COVID-19. If you do report symptoms or exposure, this information is shared with Student or Employee Health.

Failure to accurately and honestly report symptoms and/or exposures in Healthcheck, or any attempt to otherwise falsify your Healthcheck attestation, will be referred to Student Conduct or Human Resources for appropriate disciplinary action.

Click the “Passport” button after accurately completing Healthcheck to get your passport, which is good for 24 hours. Each user is assigned a unique passport number indicating their status which shows a “Clear” screen or “Not Clear” screen to display on your phone. Based on your response, UAB Healthcheck assigns you one of three categories: Least Risk, Maybe at Risk, or Greater Risk — each with its own instructions on how to proceed to campus or work.

A green passport is required to enter classes, meetings, the library, and other events on campus. Regardless of how you answer, a compliance report is sent to Student Health or UAB Human Resources.

    • Faculty and staff: DO NOT report to work until cleared by Employee Health; if you are at work, leave. Notify your supervisor that you cannot be present. Note: At this time, employees who are health care workers will not necessarily need to quarantine and will be allowed to continue to work.
    • Health care workers should continue to report to work unless otherwise instructed through Healthcheck or by Employee Health.
  • Students who have symptoms of COVID-19 or have other urgent concerns, please contact Student Health Services (205-934-3580 or 205-934-3411 after-hours/weekends).
  • Faculty and staff who have symptoms of COVID-19 or have other urgent concerns, complete Healthcheck, and notify your supervisor. Remain under home isolation until you are cleared to return to work by Employee Health.
  • If you have symptoms and are going to visit a health care provider or Student Health Services, please call first and wear your face covering.

What is UAB Healthcheck?

UAB Healthcheck is a web-based COVID-19 assessment tool for employees and students that assesses existence of any current COVID-19-related symptoms, exposure history, and testing history. The required assessment will help us all return to campus safely and help our community beat COVID-19.

UAB Healthcheck is used to manage large groups and reduce the risk of further exposure to COVID-19. UAB Healthcheck informs public health and university officials about emerging symptom hot spots before these areas have catastrophic consequences. It also helps university health officials on prioritizing participants who might need COVID-19 testing.

How does UAB Healthcheck work?

Employees and students will log on daily to share symptoms — even if you are feeling healthy — to ensure a safe entry to campus. The initial survey takes about 20 seconds to complete. Subsequent surveys take about 10 seconds to complete.

Accessing UAB Healthcheck requires your BlazerID. Employees and students are required to complete UAB Healthcheck daily.

If the survey responses indicate the user is symptom-free and has no close exposure to someone with COVID-19 illness, the individual is cleared to enter campus.

If UAB Healthcheck responses show the user is currently experiencing COVID-19 symptoms or has been exposed to someone with COVID-19, or has been diagnosed with COVID-19 in the preceding month, the user will not be permitted to enter campus until medically cleared by Employee Health or Student Health. If you have symptoms, contact your health care provider and notify your supervisor or appropriate school official. Do not come to work or report to campus (CDC isolation guidance). Employee Health (for faculty and staff) or Student Health (for students) will actively monitor your progress and follow up with you as necessary.

Why do I have to report my symptoms daily?

Because COVID-19 symptoms are a report at a single slice of time, students and employees are required to report their symptoms daily.

I'm not cleared to enter campus. Should I be participating in UAB Healthcheck?

You may begin using Healthcheck now. As you come to campus or work, you are required to take the web based training, as well update your symptoms, exposure history, and testing history through Healthcheck daily.

Why has UAB mandated that we use UAB Healthcheck?

UAB Healthcheck informs public health and university officials about emerging symptom hot spots before these areas have catastrophic consequences. It also helps university health officials on prioritizing participants who might need COVID-19 testing.

What am I required to do as a student/employee?

UAB Healthcheck is required for employees and students as part of the entry to campus protocol. It must be completed daily.

Is UAB Healthcheck secure? Is my privacy protected?

UAB Healthcheck is a HIPAA compliant platform. To access the tool, you will need to use your Blazer ID.

UAB Healthcheck does not in any way track you as a person. It is a tool for you to report COVID-19 symptoms and any exposure. It does not track your location. UAB Student Health, Employee Health and Human Relations will receive the information to ensure those coming on campus are not showing symptoms.

Will I remain anonymous?

UAB Student Health, Employee Health and Human Resources will receive your information submitted with UAB Healthcheck. The information is to ensure compliance and to identify employees or students who may need to be tested to further prevent the spread of COVID-19.

How do I access UAB Healthcheck? Do I have to use a smartphone?

To access UAB Healthcheck, you will need your BlazerID.

UAB Healthcheck is available through uab.edu/healthcheck, the UAB mobile app or the my.uab.edu portal. It is mobile-enabled so that it can be easily viewed on your phone. The site can be pinned to your smartphone home screen so that it appears as an app.

I've been participating in HelpBeatCOVID19.org. Do I have to use UAB Healthcheck?

If you have participated in the public COVID-19 assessment tool, helpbeatcovid19.org, you are still required to participate in the UAB Healthcheck. While both tools were developed by UAB experts, helpbeatcovid19.org is a public tool and not for UAB entry to campus. If you are receiving notifications for HelpBeatCOVID19.org, you can turn these off by replying STOP.

Will I receive notifications or reminders to fill out UAB Healthcheck?

There will be an option for you to receive reminders to report how you are feeling.

If you are already receiving notifications for HelpBeatCOVID19.org, you can turn these off by replying STOP.

How do I opt out of HelpBeatCOVID19.org so I only receive reminders from UAB Healthcheck?

If you would no longer like to receive text notifications from HelpBeatCOVID19.org, simply reply STOP to the next helpbeatcovid19.org text you receive. You will no longer receive notifications.

Can I participate in both HelpBeatCOVID19.org and the UAB Healthcheck?

Yes, HelpBeatCOVID19.org is a public tool that helps public health officials and researchers identify hot spots across the state. UAB Healthcheck is a tool tailored for entry to campus and is required by UAB. UAB Healthcheck will help us all unite on campus safely and avoid further risk and exposure to COVID-19.
