UAB Campus Dining
- Nutritional Information: Search nutritional information for menus from your favorite on-campus eateries.
- Myfitnesspal: A free calorie tracker program for UAB students, faculty and staff. All UAB Campus Dining menu items are searchable when you are logging your meals on the myfitnesspal smartphone app — or scan the bar codes on menus posted at each station in the Commons on the Green.
- Mindful by Sodexo: Mindful menu items focus on transparency of ingredients, delicious food, satisfying portions and clarity in message so that making mindful choices becomes second nature. Look for the green apple heart icon on the BITE menu and in the UAB Campus Dining Hall.

If you are unable to attend one of the scheduled screening events, you have the option to complete your screening through your primary care provider using the Wellscreens physician fax form. Please ensure your appointment is a preventive exam so it is recorded (“coded”) appropriately by your provider.
The following steps must be completed to receive credit for completeing a Wellscreens when using the physician fax form:
- Your provider must complete and sign the Wellscreens physician fax form.
- The completed form must be submitted to UAB HealthSmart by November 30 to receive completion credit toward any applicable incentive — no exceptions.
- You or your provider may submit the completed and signed form in one of the following ways:
- Electronically: Upload a scanned copy of the completed form through a secure REDCap link by going to https://redcap.link/uabew. When submitting electronically, please make sure your Blazer ID is entered correctly.
- Fax: Send the form directly to UAB HealthSmart at 888-257-0073.
Please note, Wellscreens physician fax forms will begin processing once Wellscreens start in October 2025. You or your provider may send in your fax form at any time, but points will not be reflected in My Health Rewards until the in-person Wellscreen screenings begin.

UAB faculty and staff enrolling in a UAB medical plan must declare whether they are a tobacco user, and a fee will be assessed accordingly. Cessation resources and discounted products are available to UAB employees; as part of the UAB employee benefits packages, tobacco cessation products are available at no co-pay with the annual limit of two 12-week cycles of treatment. If tobacco users would like the $50/month surcharge discontinued, they must complete the four-week EACC Tobacco Cessation Coaching.

Why did UAB add a tobacco surcharge?
As a world-renowned university with vital roles in preserving and improving life, UAB has a responsibility to be among leaders in health and wellness initiatives. The use of tobacco is one of the leading preventable health risks worldwide. As part of other efforts around tobacco usage on campus and in support of our wellness programs, UAB believes this is the right thing to do.

Who is required to declare their tobacco usage?
All UAB employees electing a UAB medical plan for 2026 during Annual Open Enrollment or as a new benefit-eligible employee will be required to declare tobacco usage for themselves.

Who does the tobacco surcharge apply to?
The tobacco surcharge only applies to UAB employees. It does not apply to dependents covered under the medical plan.

What is the amount of the tobacco surcharge?
A $50 monthly tobacco surcharge will be applied to employees who declare tobacco usage. This surcharge is considered a tax-sheltered amount for payroll.

What products are considered for tobacco usage?
Annual declared tobacco products include cigarettes, cigars, pipes and oral smokeless tobacco products.

If I am solely using tobacco cessation products in my effort to stop tobacco, does that count as a tobacco product subject to the surcharge?
No. Certain tobacco cessation products are available to UAB employees as part of the benefits package at no co-pay with an annual limit of two 12-week cycles of treatment. UAB recognizes these products as tools to help quit tobacco.

If I currently use tobacco and wish to have the surcharge removed, what can I do?
If you declare tobacco usage during the Annual Open Enrollment, you must register for one of the EACC Tobacco Cessation Courses by January 31. If you then subsequently complete the course, the tobacco surcharge will be removed and refunded back to the beginning of the plan year. If you are declaring tobacco usage as a new benefit-eligible employee, you must register for one of the EACC Tobacco Cessation Courses within 60 days and then subsequently complete the course.

If I register and complete the cessation course but continue using tobacco, what happens to the surcharge?
Completion of the cessation course is the only way to discontinue the surcharge. During the next Annual Open Enrollment, employees will be asked to declare their tobacco usage for the next plan year.

What if I declare myself as a tobacco user and do not register for a cessation course by the deadline, or I register and don’t complete the course?
If you do not register by the deadline and subsequently complete the course, the surcharge will continue for the plan year. If you do not register by January 31 for an Open Enrollment declaration or within 60 days of declaration as a new benefit-eligible employees, and subsequently complete the course, the surcharge will continue for the plan year.

Employees have two options for submissions:
